If online submission is unavailable, the contact page provides direct office contact options instead of collecting a message. Where an online administrative inquiry form is enabled, the name, contact details, topic, location preference, and message you provide are processed through our website host and email-delivery service for designated office and website-administration recipients. Inquiries are not restricted to your clinician. Do not use a public website form for medical or confidential case information.
Website hosting and technical information
Vercel hosts this website and processes technical information used to deliver pages and operate the service, including internet addresses, requested URLs, browser information, and request timing. Hosting and security logs are separate from patient records. Do not place sensitive information in website URLs.
Cookies and tracking
We have not enabled advertising pixels, session replay, or a third-party analytics integration in this website. Its application code does not set tracking cookies or track visitors across other websites. It does not change its operation in response to a browser’s Do Not Track setting.
