Open Choice’s EZ-NET provider portal to sign in, or request portal access through Choice’s account-request page.

Open EZ-NET Request portal access

Registration starts with your practice’s Tax ID on Choice’s website. Available access and account approval are determined there. Portal registration is separate from joining Virtue IPA.
